Shuts down when taking pictures and it's super annoying!!!!!

For the last month or so when completing a task when I try to take a photo in any section of the task, the camera pulls up and I can snap the photo but when I hit the check to submit, the screen goes blank and goes back to the beginning of the task and will not submit photos.
